Insurance claims can be processed quickly. Follow these instructions:

  • Complete and sign the medical claim form, indicating whether the doctor/Hospital has been paid. Once the following steps have been taken, please send claim form and attachments to: CISI, River Plaza, 9 West Broad Street, Stamford, CT 06902-3788.
  • Attach original itemized bills and/or receipts for all amounts being claimed. When reimbursement of an expense is approved, it will be made to the provider of the service unless the receipt of the bill is noted as having been paid by you. Payment will be made in U.S. dollars unless otherwise requested.
  • For foreign claims, provide a translation including all of the following elements: date of service, details of injury/illness, when injury/illness began, diagnosis, description of procedures performed on patient by doctor and approximate value (in U.S. $) of the currency that appears on the bill(s).
  • If payment is to be made to the provider of the service, the provider's name, address, telephone number and taxpayer identification number (if the provider is in the U.S.) must be included on the bill receipt. If payment is to you, it will be mailed to your U.S. address unless otherwise requested.
