Yes, you may request an enrollment template by contacting the Plan Administrator at email@example.com. Enrollments will be processed in-house and all materials will be mailed to the leader/contact of the program.
CISI has access to both the domestic and international insurance markets. Our participants travel to more than 100 countries annually - from Costa Rica, to Italy, to Brazil, to Egypt, to China, to the United States and more. Our own international and sister offices give CISI direct connections in the United States, the United Kingdom, Germany, South Africa, and Australia.

To use our insurance plans overseas, participants should present their CISI insurance ID card to the medical provider at the time of service. Foreign providers can contact the assistance service toll-free to verify eligibility and / or benefits 24-7-365. If a participant is still required to pay for any medical services, medicines, or equipment out-of-pocket, the participant should submit those bills and/or receipts to CISI for reimbursement, along with a completed claim form available through their myCISI portal.
Participants can access an online database of providers in the U.S. who participate with the First Health Network and are located in their area. For help locating a provider overseas, participants can contact the assistance service by calling the collect number on their insurance ID card and in their myCISI portal.
If you have a customized group insurance plan with CISI, the best method for enrollments request is via the myCISI online portal.
Group insurance plan participants may also self-enroll via your customized myCISI participant enrollment portal.

To enroll for up to one month of coverage either before the program start date or after the program end date, participants will need to enroll in our individual study abroad plan.
The group insurance plan billing contact will be invoiced for enrollments. Payment is due upon receipt of the invoice. Acceptable payment methods include: check, money order, Visa, MasterCard or American Express.
